We're welcoming new patients and would love to have you join the Sleep Therapy Solutions family. Here's everything you need to get started.
Getting Started
The process is straightforward. Here's what to expect from your first call to receiving your equipment.
Talk to your physician or sleep specialist about your symptoms. They will order a sleep study and, if needed, prescribe DME equipment.
Ask your doctor to send a referral to Sleep Therapy Solutions. We can also coordinate directly with your physician's office on your behalf.
Our team contacts your insurance carrier to verify your benefits and obtain any required prior authorizations — at no cost to you.
Once everything is in order, we deliver your equipment, provide hands-on setup and training, and follow up to make sure therapy is going well.
Required Documentation
To get you set up as a new patient, we'll need the following items from your physician's office. We can coordinate with your doctor directly.
A signed prescription or order from your physician authorizing the durable medical equipment you need.
Results from relevant diagnostic testing, such as a sleep study or oxygen testing, as required for your equipment.
Your personal demographic details and current insurance information so we can verify your coverage.
Clinical notes from a face-to-face visit with your doctor discussing the medical necessity for sleep testing and equipment.
We coordinate with your doctor. You don't have to gather all of this yourself. Once you contact us, our team can reach out to your physician's office directly to collect the required documentation. Call us at (330) 655-0630 to get started.
